You verify that you are the current subscriber or owner of any telephone number that you provide to us. You acknowledge that by voluntarily providing your telephone number(s) to LifeCap, you expressly agree to receive prerecorded voice messages and/or autodialed calls from us and our third party service providers, including alerts about your relationship with LifeCap, and other products or services offered by LifeCap or its affiliates and confirm that you are over the age of eighteen. You also agree that LifeCap may obtain, and you expressly agree to be contacted at, any email addresses, mailing addresses, or phone numbers provided by you directly at any time or obtained through other lawful means, such as caller ID capture. You may also receive SMS text messages related to customer care, transactional activities and promotional/marketing offers. You agree to receive the communications outlined above even if your policy does not qualify, except if you opt-out, as provided below. Consent to receive automated marketing calls/texts is not a condition of qualifying your policy. LifeCap or its affiliates may call or text you even if your telephone number is registered on any state or federal Do Not Call list. You may incur charges for these calls from your telephone carrier. LifeCap and its affiliates are not responsible for these charges.

Please be aware that LifeCap employs artificial intelligence (AI) systems and virtual agents for some interactions. The use of AI or automated agents will be disclosed at the beginning of the call. You acknowledge that AI systems may occasionally produce errors or inaccuracies despite our efforts to ensure accurate and reliable information.

Should you choose not to accept our services at this time, we may still contact you in the future with communications and offers related to our products or services as part of our ongoing relationship and commitment to service excellence.

You may opt-out of automated calls or text messages at any time.

To opt-out of automated voice calls, you may, among other reasonable methods, (i) provide us with written notice revoking your consent; (ii) in that written notice, you must include your full name, mailing address, and the specific phone number(s) for which you wish to cease telephonic communications; and (iii) send this written notice to Attention: LifeCap, 100 S.
